Workshop Request Form

Request a TLC workshop at your school or as a Field Trip to visit DE at the U of L. 

Before you start, check the workshop availability calendar for available dates. The listed dates/times apply to workshops booked both as a field trip and if we travel to you from Oct-April due to staffing.

School Info
Please provide a number that you can be reached the day of.
Please let us know if you are travelling to the UofL by bus, and if that bus will require a parking pass to park on campus.
Class #1
Provide the dates and prefered 1-hour time block for your workshop. Refer to the availability sheet to help you find an open time.
Extend your field trip by adding a second workshop for only $75. A break will be incorporated in between the two workshops for your class to run around and have a snack.
If your school is outside a 30km radius from the U of L, we ask that you book a second workshop in the same morning/afternoon and date. Please talk to your fellow teachers.
Class #2

Additional Information
All invoices will be sent by email. Invoices will be sent approximately 1-week after the workshop is complete.
