Police Information Check Form
This form can be completed by students in the Faculty of Health Sciences who wish to receive a reduction in cost for a Police Information Check for their practicum or internship placements.
This form is to be brought to students local policing service when they initiate the police check process.
Certain programs require students to submit original copies of their Police Information Check to the University of Lethbridge; please bring to M3080 or mail your Criminal Record Check to:
