Accommodated Learning Request
- Visit the Accessible Learning Centre website for details on available accommodations.
- Best completed by your family doctor if possible.
Allergy Injection
- For ongoing allergy injection therapy.
General Check-Up / Complete Physical
- Comprehensive exam including inspection, palpation, and assessment.
Head Injury
- For recent head injuries. Please contact the Health Centre directly, as multiple providers may be involved for proper assessment.
Illness
- For acute symptoms such as sore throat, flu, or cough.
Initial Mental Health Appointment
- For students seeking mental health support at the Health Centre for the first time.
- Bring any prior documentation from other providers.
- Services may include:
- Starting or continuing medication
- New diagnosis
- Referral to psychiatry or therapy
- Assessment forms will be emailed the day before your appointment and should be completed in advance.
Mental Health Follow-Up
- For ongoing mental health care at the Health Centre.
Other Concerns
- Examples include:
- Acne or other skin concerns
- Lab requests or review
- Allergies
- Injuries, strains, or sprains
- Eye, ear, or nasal issues
Pelvic Exam
- For pelvic exams/concerns and Pap tests.
Prescription Requests
- New: Non-mental health prescriptions.
- For new mental health medications, select Initial Mental Health Appointment.
- Refill: Bring a prescription list if the Health Centre has not previously filled the medication.
STI Testing
- Symptomatic or asymptomatic testing and test-of-cure.
Third-Party Medical Forms

- Call the Health Centre staff for more information on fees.
Urinary Tract Infection
- Testing and treatment for bladder infections.
Wart Treatment
- Ongoing treatment or referral to podiatry.
