The University of Lethbridge Health Centre is committed to providing a safe, confidential environment. We collect only the personal information needed to provide clinical and mental health services, manage your care, and support essential operations such as appointments, lab reports, statistics, and billing. Your Alberta Health Care Insurance number may be collected as authorized under the Alberta Health Care Insurance Act. All information is managed in accordance with provincial health legislation.
Use and Disclosure of Information
We collect basic service statistics and take every measure to protect your privacy. In rare situations, we may be legally required to share information—such as when a child is at risk or when there is concern for your safety or the safety of others.

Medical Records
Your medical records are kept separate from academic records. They are retained for 10 years after your last appointment, then securely destroyed.
Records will not leave the Health Centre without your written consent unless required by law, including under:
De-identified records may be used for general research.
New Patient Registration Form
If you are a current University of Lethbridge student who has never visited the Health Centre, please complete our New Patient Registration Form.
This form provides the information required to create your chart and book your first appointment with a Health Care Provider.
Medical Release Form
Treatment records are intended for use by physicians, psychiatrists, psychologists, and other recognized health professionals. Patients cannot directly access their own records but may authorize a qualified professional to obtain them on their behalf.
To request a release of information, complete the Consent to Release Form and fax it to 403-329-2466 or email it to health.centre@uleth.ca. A copy of your records will be sent to the authorized professional.

Alberta Health Care Application Form
Should you desire to apply for an Alberta Health Care Card or need to update your personal information linked to your card you may do so here. International students are welcome to visit the Health Centre for assistance with their application.
On Campus Incident Report Form
Injuries or near miss events must be reported to Risk and Safety Services by completing a Campus Accident/Injury Report wtihin 24 hours of the event.
Basic Health Care
In Canada, basic health care is provided through provincial health insurance plans. All University of Lethbridge students must have basic health coverage. The Alberta Health Care Insurance Plan (AHCIP) covers services such as physician visits, hospital care, surgery, and medical tests.
If you are from another Canadian province, you may continue using your provincial health card while studying in Alberta. If you do not currently have basic health coverage, the Health Centre can assist you with applying for AHCIP. International students are encouraged to visit the Health Centre for help with their application.
Please always carry your provincial or Alberta health care card, along with photo ID.
If you need a replacement Alberta Health Care card, you can request one by contacting Alberta Health directly:

You may also visit any participating registry office to submit a replacement request.
Extended Health Insurance
Extended health coverage pays for services not included under basic provincial plans, such as ambulance services, prescription medication, and dental care. Full-time students are automatically enrolled in the Students’ Union Extended Health and Dental Plan, with associated costs included in tuition.
For details on the plan—or to opt out if you already have comparable coverage—contact the Students’ Union Office (SU180).
International students require two types of health insurance while studying in Canada: a basic health plan and an extended health plan.
Basic Health Insurance
Option A: Alberta Health Care Insurance Plan (AHCIP)
(For students staying in Alberta for more than 12 months)
All students must have basic health insurance. The Alberta Health Care Insurance Plan (AHCIP) covers physician visits, hospital care, surgery, and medical tests.
• If you do not already have basic health coverage from your home country and will be in Canada for more than one year, you must register for AHCIP.
• There is no cost for this plan, but registration is required.
• Please visit the Health Centre Team within your first two weeks in Canada for assistance in completing your application.
Option B: Travel Health Insurance
(For students staying less than 12 months)
Students staying in Alberta for less than one year are not eligible for AHCIP.
You must obtain travel health insurance from your home country or province.
If you will be in Canada for more than a year, AHCIP is typically more affordable than maintaining long-term travel insurance.
Extended Health Insurance
Extended coverage pays for services not included under basic provincial plans, such as ambulance services, prescription medication, and dental care.
• Full-time students are automatically enrolled in the Students' Union Extended Health and Dental Plan, with costs included in tuition.
• This extended plan is not available to EAP (English for Academic Purposes) students.
The University of Lethbridge Board of Governors requires all International Visa students to have valid health insurance while enrolled.
