U of L Ed 2500 Practicum Recognition Option Form (PROF) Fall 2024
Thank you for hosting an Education 2500 student this semester. Please fill out this form for each student teacher if applicable. In order to issue your recognition option, please complete and submit the PROF prior to October 30, 2024 in order to meet the November 8 payroll deadline.
NOTE: If the PROF form is not submitted the honoraria will default to the waive and the funds will be redirected to support Faculty of Education practicum students.
