Payroll Deducation Request Form

Gifts made by payroll deductions are tax-deductible, however, no individual tax receipts will be issued. Charitable donations made by payroll deduction will be included in Box 46 on your T4 summary at the end of each tax year. Please fill out this form to sign up for payroll giving.
