Technical Service Fabrication Request Form

This webform is used to submit a work request for fabrication services provided by Technical Services in the Faculty of Arts and Sciences at the University of lethbridge. The completed form will be submitted to the Technical Services Machinist for consideration. A copy of the work request will be emailed to the applicant for their own records.

The FOAP is necessary for billing for materials required for the project
This can be used to upload a drawing or schematic in one of the following formats: gif, jpg, png, bmp, tif, eps, pdf If you would like to upload a different file type or multiple files please use a zip file.
