Online Confidentiality Form
In response to recommendations from the Office of the Auditor General (OAG), several departments worked together in 2015 to provide a process for ensuring employees are aware of their responsibility for managing confidential information. In the past, many new employees signed a paper form at the beginning of their employment, but there were inconsistencies and no requirement for an annual update to reaffirm responsibility for managing confidential information.
In order to meet the expectations of the OAG and to make the process as quick and easy as possible, an online process was developed. The Online Confidentially Form provides employees with the ability to acknowledge their awareness and understanding of expectations for managing confidential information on behalf of the University. Acknowledgements of expectations regarding confidentiality for faculty members is handled separately.
Some helpful information:
- The online form has been simplified to ease the acknowledgement process.
- This process is similar to but separate from the Conflict of Interest and Commitment Declaration process that takes place in January of every year.
- This annual process takes place each October. Each employee will receive an email with instructions shortly.
